Let's take a look at the history of a document...

The user seems to have had trouble working with this document which he first added, then deleted, the added back. Finally he purged the file.

The first hint of trouble appears with the next event which shows that he managed to persuade the system to delete (logically) a document that has been (supposedly) physically removed.

After which he added it back and then purged it again.

Despite which, SourceSafe is showing it in the Explorer listing!
